  15. How many e-mails did you send, or reply to, that were not about work related issues?
  16. How many times were you distracted by a colleague during the day?
  17. How many times did you distract a colleague during the day?
  18. How many times did you think about some personal issue, person or activity?
  19. How much time did this take from the working day?
  20. Did you have a personal problem that was playing on your mind?
  21. Was there something personal that you forgot to do, that you thought about?
  22. Do you have a personal problem at the moment that is playing on your mind?
  23. How many times did you start to work or do something and didn't feel like doing it at that time?
  24. How many times did you stop work for a breather, even if it was just for a minute or two?
  25. How many minutes did that take out of your day?
  26. Was there something that you forgot to do yesterday?
  27. Did you find yourself watching the clock and looking forward to your coffee break, lunch break, or going home?
  28. Did you stop before time and talk to colleagues for a few minutes at the end of the day?
  29. If so, how many minutes did that take?
  30. Do you have anything about work or life in general that is worrying you at the moment?
  31. If so, how much time do you think you spend worrying about this?
  32. How much of your day was wasted by inactivity or inability to concentrate fully in any given moment?
